“Where do I get a MA Clinical Application packet?”
The application packet is now available entirely in electronic format. You should review the application packet instructions and print/complete the required forms.
“What do you want in your application packet?”
Only include those items in your application packet listed under application packet instructions. Do not include: letters of recommendation, copies of degrees/certifications, records of immunization/medical examinations, BLS card, or any other correspondence not specifically requested by a MA advisor.
All items completed for the application packet should be placed in a single large envelope and addressed to:

You should begin working on application packet requirements during the fall semester preceding the spring application deadline of April 1.
