Equipment Reserve/Checkout Form

Faculty and Staff may reserve Library equipment online for on-campus usage only using the form below. Faculty and Staff needing to checkout equipment for off-campus use must visit the Library. Additional paperwork requiring several signatures is needed for off-campus checkout. Online equipment reservations must be received at least 48 hours in advance of the date the equipment is needed. Online reservations are subject to equipment availability. All reserved equipment must be picked up at the Library circulation desk.
